Founded by Nurse Practitioner Shalonda Jacobs in November 2023, our practice was built on a deep commitment to restoring health, promoting wellness, and sharing Christ’s love through every interaction.
From our humble beginnings, we’ve grown quickly into a trusted name in advanced wound care and mobile health services across Southeast Texas. With convenient clinic locations in Houston, Beaumont, Nederland, and Jasper, and a service area that spans over a dozen counties, we’re proud to make high-quality care more accessible than ever before.
